Important information about Medicare coverage, returns, exchanges, and our commitment to your care

For Our Medicare Patients

If you are currently on Medicare or a Medicare HMO that we accept, and are diagnosed with Diabetes you may qualify for one pair of Diabetic Shoes and three pairs of Diabetic inserts each Calendar year.

Wearing properly fitted shoes and using Diabetic Inserts is one of the most important things an at-risk Diabetic Patient can do to protect their feet. CustomFit ProLabs is here to help you do that.

Qualification Requirements

To qualify for Diabetic Shoes and Inserts, you must have a recent exam (Within the last 90 days) with the physician (MD or DO) who is managing your Diabetes. That Doctor will have to fill out a "Statement of Certifying Physician Form" for Medicare. Your Podiatrist (DPM) can write you a prescription but only the Doctor managing your Diabetes can fill out the required Medicare form.

Our Process

Once we have evaluated you here at CustomFit ProLabs or at your home, and have all the required documents, we can fill from stock or order and dispense your new Diabetic Shoes and Inserts. This process depends on how long it takes to get back all the required documentation from the Doctor.

Available Options

Your Doctor may prescribe either Extra Depth Shoes or Custom Molded Shoes, along with pre-fabricated or custom inserts for your shoes, depending on your foot type and Diabetic conditions. Custom molded inserts are fabricated right here in our lab specifically for you using a foam impression or a digital scan of your feet.

Insurance Coverage

Medicare will pay 80% of the allowable amount and if you have a secondary it can be billed for the remaining 20%. If you have a Medicare HMO they may cover the Diabetic Shoes and inserts in full or you may have a copay or co-insurance, We will check your insurance for you and advise you of the status for your particular plan.

Returns & Exchanges

Need to return or exchange something? We can accept returns for most merchandise for up to 30 days from the date of receipt. And we will be pleased to issue an exchange or refund of your purchase. As you may expect, we ask that you return your purchase in unused, resellable condition with the original packaging intact.

30 Days Return Policy

  • To be accepted for return, items must be in original purchase condition including the original packaging with all parts and accessories.
  • Any item received with tags must be returned with tags intact and attached.
  • All packaging must be unmarked and not defaced in any way.
  • After merchandise is accepted for refund, a credit for the purchase price will be issued in the same form of payment as your original purchase.
  • We do not accept returns or exchanges for clearance items or special orders.

Footwear Returns

Footwear is to be its original, unworn condition. Be sure to try on footwear inside on a carpeted surface before wearing outside. We do not accept returns or exchanges for clearance, liquidation, and special ordered items.

Socks/Hosiery Returns

Socks or Hosiery must returned in its original unworn condition. We do not accept returns or exchanges on final clearance items.

How to Return an Item

By Mail:

Please complete the Return /Exchange form on the form provided with your order, fill out the form completely, and form must be included in your returned package to receive full credit.

  • Send package back using the shipper of your choice. Insure package for the full value of the merchandise.
  • Keep your shipping receipt until your account is credited or you've received a refund check if paid by check or money order.
  • Unfortunately we are not responsible for items lost or damaged in transit.
  • Credit Card purchases will be credited to the original purchased credit card account.
  • Cash and Check purchases will be refunded by mail issued within 10 days after refund is received and accepted.
  • We do not accept returns or exchanges on clearance items.

Exchanging an Item by Mail:

If you would like to exchange an item please complete the return/exchange form that accompanied the packing slip included with your order. Indicate the item(s) being returned for exchange as well as the new item(s) desired.

  • If you would like to exchange items for other items with a higher cost, please include payment by U.S. money order or we will be glad to charge a credit card you indicate.
  • If you would like to exchange an item for a lower priced product, following inspection of the received items, we will refund the difference to you in accordance with our return policy.
  • Please allow 15-20 business days for us to receive your item(s) returned for exchange, to process your exchange, and for you to receive item(s).

Gift Returns:

Unfortunately we will only be able to provide a refund directly to the purchaser of the gift. If you would like to exchange your gift for another item, we will be glad to do so. Please refer to our exchange policy in the section, exchanging an Item by Mail.

Incorrect/Damaged or Defective Merchandise Received:
